In coordination with the Registered Nurse and the interdisciplinary team of the department, performs mental health counseling, and provides behavioral intervention based on assessed needs for patients. Routinely performs patient care duties. Provides a safe and therapeutic environment and may assist in the admission, provision of patient care, discharge, and transfer of patients. Provides all services following established hospital/departmental policies and procedures.


Monitors patient's mental and social functioning and patient's response to treatment and report changes to RN/MD to assure optimal patient outcomes. Actively provides input on patients in shift report. Documents in the patient chart significant patient care observations, as observed through group, individual sessions, and within the milieu.Participates in the development and ongoing review of the patient's treatment plan and progress towards goals. Records patient observations, interventions, and outcomes in progress notes and flow sheets.


The Mental Health Counselor works as part of a multi-disciplinary team, to provide behavioral intervention based on assessed needs for patients with related psychiatric conditions. Is knowledgeable about the assigned patient's treatment goals and intervenes as identified in the treatment plan. Is able to implement assigned aspects of the treatment plan under supervision. May facilitate or lead group psychoeducational sessions in the milieu and documents the sessions. Effectively sets limits in the milieu, both with consistency and flexibility when necessary. Maintains a structured, therapeutic environment that addresses the age, developmental and psychological needs of the patient and includes individual time with assigned patients and conducting psycho-educational and other group activities per the unit schedule.


Assists in patient admission, transfer, and discharge procedures. Participates in the care needs of all inpatients as requested or directed by Unit RN. Carries out the following tasks, according to policy and procedures: Vital signs (TPR and BP), specimen collections, bathing, and personal care when needed, intake and output, weights, carry out special precautions (i.e., seizures, elopement, suicide), serving and collecting meal trays and snacks, lifting and moving patients, linen distribution. Utilizing the least restrictive interventions assists in the management of aggressive or destructive patient behavior according to the policy and procedures up to and including physical intervention and/or the application of restraints. Facilitates patient/family participation in self-care based on the age and the developmental status for both pediatric and adult populations. May complete assistive duties such as answering phones, washing the patient's clothes, accompanying patients for off-unit tests, and running errands.Provides a safe, clean environment thatprotects the patient's rights and includes completion of levels of observation and checks of the patients/surroundings and records the results on flow sheets. Guides and assists patients in adhering to unit policies and procedures. Integrates patient safety principles into all aspects of the patient's plan of care. Supervises patients in the care of their room and unit environment.


Participates in departmental and system-wide performance improvement and quality initiatives. Maintain appropriate charting documentation per Loyola Medicine and department policy. Documents in EPIC all pertinent patient information by the close of each business day. Properly codes for relevant and approved services. Reports potential risk or quality management issues to the department manager or the director. Evaluates environmental safety and provides an environment that is safe based on the age and the developmental status for both pediatric and adult populations served to maintain compliance with National Patient Safety Goals.


Remains informed of developments in area of expertise.Seeks opportunities to meet learning needs through seminars and literature, including mandatory education requirements.Conforms to the established Standards of Behavior. Demonstrates capability to adapt and change work processes and role responsibilities or activities to improve service delivery. Establish effective networks with colleagues inside and outside LUHS. Complies with the skills, competencies, and educational requirements related to the job and the department.

+ nRequired: Level of knowledge equivalent to that ordinarily acquired through completion of two (2) years of college with course work in psychology or a related field preferred of the equivalent combination of education and experience.
