Keystone Health, located in Eagle, is currently searching for a Certified Medical Assistant to join our Chronic Care Management team. The ideal candidate is reliable, detail-oriented, and self-motivated. This position involves monitoring patient health status, following up on directed care by providers, and orchestrating seamless care for our patients. The CCM program collaborates directly with... providers in a variety of healthcare settings to identify patients with chronic conditions and alleviate gaps in care. Chronic Care Managers interact with patients and families via phone calls, emails, and visits to ensure comprehensive care.
