The Waiver Provider Care Coordinator is responsible for performing oversight duties as set forth in the Medicaid Waiver HCS and Texas Home Living programs, as well as the IDD Provider principles and guidelines, regulations, and LRCC policies and procedures for up to 50 individuals receiving waiver services. The Waiver Care Coordinator is responsible for developing, organizing, implementing, and monitoring an array of supported services for people with intellectual and developmental disabilities, and related conditions. Provides harmonious coordination of services with other LRCC departments and outside service providers for caseload assigned. PCC performs a wide variety of documentation and communication with external and internal customers. This position requires frequent travel between individuals homes, community, and LRCC provider services in their personal/company vehicle. Assumes a 24 hour responsibility and will receive a monthly on-call stipend. PCC may provide training in the home or in the community to assist individuals with developmental disabilities to gain more independence. Position ensures that individuals served receive the life skill services identified in the person directed plan to meet their needs, desires and personal outcomes. Responsibilities include completing assessments and identifying needs; providing transportation; and documentation of services provided. PCC uses structured intervention techniques implementing the most effective, but least intrusive methods possible to help the consumer, who are enrolled through waiver programs to learn the essential soft and hard skills of the skills necessary to progress and/or maintain life skills. Serving as telehealth presenter, within scope of IDD/MH treatment during clinical telehealth, including tasks such as establishing and maintaining video conference connections, making introductions between new individuals and doctor. Follow instructions of the doctor to facilitate an effective clinical experience for all. Duties necessitate extensive travel within the local service area. Position requires 25 hours of direct care time be provided to individuals weekly. All other duties assigned by the supervisor.

8-5 Full-Time, 40-hours per Week (Flex)
$15. 00-$17.00 per Hour, Plus $100 per Month On-Call Pay
Requires a high school diploma or GED. Must have a valid Texas drivers license and acceptable driving record, as well as personal automobile liability insurance as required by the state of Texas.
Proficient written and verbal skills, ability to supervise the work of others, ability to work in an environment with minimal supervision, ability to assess and coordinate individual needs and follow up as needed, provide guidance and training to staff, monitor program effectiveness with knowledge of community resources. Proficient computer skills, with knowledge of electronic charting, preferred.
