Summary The Albany VAMC is looking to fill 1 Registered Nurse - Home Based Primary Care position. The HBPC staff nurse is a registered professional nurse with clinical knowledge and experience to function as an effective member of an interdisciplinary team providing primary care in the home. You will be required to have a valid unrestricted driver's license and to travel within the Albany catchment area for this position. Responsibilities VA offers a comprehensive total rewards package: VA Nurse Total Rewards The staff nurse interprets the philosophy and goals of Nursing Service to others in the work setting; functions within established guidelines and demonstrates the ability to work effectively and develop therapeutic relationships with clients, their caregivers and other professional and supportive personnel who provide client care. The duties of the staff nurse include but are not limited too - Performs complex, comprehensive physical, psychosocial, and spiritual assessments Completes and documents nursing assessments to identify client's needs in a self-directed manner including standardized fall risk tool, medication reconciliation and education at initial intake. Reassesses on a continuum, recognizing changes in client's individual needs, and re-establishes priorities for care. Develops the initial nursing plan of care and updates the plan using information provided from team assessments, in a self-directed manner upon change in patient condition and at minimum of every 90 days. Includes client/family in establishing realistic, measurable health goals, and planning care. Addresses patient safety needs, following National Patient Safety Goals when communicating patient care needs to other nursing staff and disciplines. Implements nursing interventions and provider orders in a timely and accurate manner for patients with complex nursing care needs. Initiates and documents teaching based on patients/family's identified needs, strengths, weaknesses and deficits. Evaluates effectiveness of education based on plan of care. Fosters critical thinking and self-direction in staff by guiding them through the steps of problem solving which results in staff's ability to independently and competently manage similar situations in the future Evaluates client status and response to care. Recognizes actual or potential changes in clients' condition and response to treatment, initiating appropriate action and revising the care plan accordingly Documents care given in the medical records in a consistent, accurate, concise and timely manner Supports and Enhances patient self-determination. Serves as a resource for patients and staff in addressing ethical issues : Identifies and assesses resource utilization and safety issues, taking appropriate action. Acquires knowledge and skills to maintain expertise in area of practice. Participates in educational activities to improve clinical knowledge and enhance role performance Evaluates practice of self and others using professional standards, relevant statutes and regulations. Takes action to improve performance Initiates/participates in quality improvement activities that result in improved outcomes. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Minimum of 1 year RN experience in home, acute, or palliative/hospice care Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
