Job Description:

All staff members at Yakima Valley Memorial must be in compliance with the Vaccination Policy, which includes the COVID-19 (SARS-CoV-2) vaccine, by their first day of employment.

Actual pay rate will be calculated based on relevant years of experience and education/certification requirements.

ESSENTIAL RESPONSIBILITIES

  1. Works independently in inpatient hospice settings; home settings; nursing home settings; assisted living settings coordinating care with caregivers and family members

  2. Uses teach back skills, motivational interviewing and other tools to engage the patient and family in goal setting and self-management.

  3. Provides education, treatment and emotional support to help patients impacted by injuries and illness.

  4. Reviews Roadnotes, documentation, client activity, and inpatient module and provides patient care based on these patient specific elements.

  5. Communicates and/or coordinates with the Provider and interdisciplinary team in planning and achieving patient goals involving the patient and family in the care planning process.

  6. Answers and triages Home Health and Hospice patient calls and assesses for in-person visit necessity.

  7. Documents on laptops at point of care as outlined in policies/procedures.

  8. Practices time-management skills and achieves agency productivity metrics.

QUALIFICATIONS AND REQUIREMENTS

Education: Associate's degree in Nursing required; BSN preferred.

Experience: Two years acute care or nursing home experience preferred.

Licenses/Certificates/Registration: Washington State (DOH) Registered Nurse licensure required. American Heart Association BLS required. Valid driver's license and proof of automobile insurance.

Additional requirements: Cell phone for use during working hours; car for travel to and from home visits during shift.
