The primary purpose of this position involves case management of assigned caseload. The Social Worker is a member of the interdisciplinary treatment team, which is responsible for care and treatment of patients admitted to JIRDC. This position assists individuals and their families with admissions and discharges in addition to services at the facility. Responsibilities include performing biopsychosocial assessments and developing treatment plans with other team members. This involves the provision of social work services in the context of a multisystem approach, including provision of services to the family and orchestration of those services and resources which are needed to meet the individual's needs.
