Support Coordination Team Leader (Developmental Disability Specialist III)

Job Description:

Responsible for programmatic, fiscal, clinical, administrative, and supervisory functions within his/her department. Supervises and trains staff, plans interventions to meet client needs, and provides staff assistance with crisis intervention. Identifies, advocates, and secures needed support services for individuals living or working in the community. The team leader has both field and office duties. Additionally, he/she has knowledge of personnel regulations, operational procedures, and Medicaid Waiver regulations. Ensures that support coordinators are in compliance with Medicaid Waiver requirements. Will complete utilization reviews, covers in the absence of a support coordinator, and manages or assists with client emergencies. Must be fully capable in using all facets of an electronic health record.

For Positions in CSB Support Coordination:
  • Provides clinical consultation, supervision, evaluation and oversight to lower level specialists;
  • Makes clinical assignment of cases based on assessment of client needs, staff experience and caseload levels;
  • Consults with professionals on a variety of problems including delivery of needed services for clinically complex cases;
  • Ensures the proper development, implementation and review of individual service plans;
  • Communicates agency mission to subordinate staff and advises on its implementation;
  • Ensures compliance with Medicaid regulations;
  • Prepares and/or reviews administrative reports, forms, and files to ensure compliance with program licensure and certification requirements;
  • Provides liaison to community agencies to facilitate communication and service provision for consumers requiring multiple services;
  • Acts as an advocate for clients;
  • Represents the agency on regional, county, state and multi-agency committees;
  • Investigates, analyzes, and reviews issues affecting service delivery;
  • Ensures adherence to, and implementation of, federal, state, local, and agency regulations and licensure standards;
  • Represents service area director in management team and other meetings.
  • Maintains ongoing communication with families, landlords, county programs and agencies, and state agencies.

  • Extensive knowledge of principles, practices, methods, and techniques related to treatment of persons with intellectual disability or other developmental disabilities;
  • Knowledge of major methods of treatment of developmentally disabled individuals;
  • Knowledge of intake procedures, social history development and interviewing techniques;
  • Knowledge of community resources, including relevant public and private organizations;
  • Knowledge of psychiatric, psychological, and/or sociological terminology and concepts;
  • Knowledge of federal, state, and local laws and regulations, as well as issues relevant to human rights, mandated reporting, and skill in applying this knowledge to a variety of situations;
  • Ability to supervise and train service professionals;
  • Ability to conduct performance evaluations and implement personnel procedures;
  • Ability to successfully perform as a team member/leader;
  • Ability to recognize external behavioral symptoms of developmentally disabled individuals who may concurrently exhibit signs of mental illness or substance abuse;
  • Ability to establish rapport and maintain effective relationships with individuals;
  • Ability to develop and maintain effective working relationships with subordinates, co-workers, public and private sector organizations, community groups and the general public;
  • Ability to recommend alternatives to allow for individual choice in services;
  • Ability to communicate effectively, both orally and in writing;
  • Ability to maintain records and prepare reports.
Graduation from an accredited college or university with a bachelor's degree in psychology, social work, or a related field; plus five years of experience working with individuals with developmental disabilities;
Graduation from an accredited college or university with a bachelor's degree and a valid Qualified Developmental Disabilities Professional (QDDP) certification plus five years of experience working with individuals with developmental disabilities.  

Masters can be substituted for 1 year of experience; or graduation from an accredited college or university with a doctoral degree in psychology, social work, or a related field

Contingent upon the area of assignment, some positions within this class may require specific certification(s) prior to employment and/or the ability to acquire necessary certification(s) within a specified time period following employment. The certifications are identified in the position description and employment advertisement.

  • Valid motor vehicle driver's license with fewer than six demerit points (or equivalent in another state) and maintained throughout employment with CSB.
  • First Aid and CPR certification within 3 months of hire and maintained throughout CSB employment.
  • Qualified Developmental Disabilities Professional (QDDP) certification required within 1 month of hire.

The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.

This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies).  

All newly hired employees are required to be fully vaccinated against COVID-19 (two weeks after the last required dose) as a condition of employment or obtain approval of a medical or religious exemption prior to their start date. Proof of an exemption or vaccination status will be required during the pre-employment onboarding process. New employees who obtain an exemption from the vaccine mandate for medical or sincerely held religious beliefs may be subject to a weekly testing requirement. Vaccinated employees and employees with a medical or religious exemption will complete the attestation online on their first day of employment or shortly thereafter.

  • At least three or more years of supervisory experience.
  • Experience with and understanding of Medicaid and Medicaid Waivers.
  • Experience with electronic health records systems

Ability to communicate effectively orally and in writing and use keyboard driven equipment. Ability to drive to numerous locations in the community during the course of the day (include Residential, Vocational, and other facilities). All duties performed with or without reasonable accommodations.

Panel Interview and may include a practical exercise.
