Responsible for the coordination of clinical activities of the Wound Care Center.  These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through the use of hospital and nursing standards. Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patient care. Provides efficient and effective care with the guidance of the physician during the clinic sessions, attends the clinics where scheduled and is dependable.  Able to manage a number of patients at the same time with high energy and organization. Collaborates with other health care providers, wound care center Physicians, Program Director, Clinical Manager and Medical Director, regarding clinic and patient needs.



Essential functions are those tasks, duties and responsibilities that comprise the means of accomplishing the job’s purpose and objectives. Essential functions are critical or fundamental to the performance of the job. They are the major functions for which the person in the job is held accountable. Following are the essential functions of the job, along with the corresponding performance standards.


1. Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patient care.



2. Provides efficient and effective care with guidance of the physician during the clinic sessions,  attends the clinics where scheduled and is dependable.  Able to manage a number of patients at the same time with high energy and organization.

  • Performs admission assessment in accordance with specific unit standards.

  • Identifies and documents nursing diagnosis on patient’s plan of care at pretest appointment and on day of admission.

  • Identifies and documents patient/family/significant other teaching needs at pretest and on admission.

  • Identifies discharge planning needs of patient/family/significant other at pretest and on admission.

  • Verification of patient identity.

  • Checks current history and physical, laboratory results, the execution of preop/preprocedure orders and results of ordered diagnostics, if applicable.

  • Preoperative plan of care of completed prior to commencement of the procedure.

3. Develops, discusses, and communicates a realistic problem list (plan of care) in collaboration with each patient/family/significant other to address all identified needs.

  • Develops patient/family/significant other teaching and discharge plan as per unit standard.


4. Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the patient.

  • Demonstrates knowledge of policies and procedures for administering, transcribing, and recording medications.

  • Demonstrates knowledge of commonly used meds: action, dose, and side effects.

  • Observe five Rs for administering meds: right patient, right drug, right dose, right method, and right time.

  • Demonstrates proper procedures in all methods of medication administration.

  • Identifies priorities and executes nursing care appropriately.

  • Provides emotional support and comfort measures.


5. Collaborates with other health care providers, wound care center Physicians, Program Director and Medical Director, regarding clinic and patient needs.

  • Reassessment of plan of care is done per Wound Care Center standard.

  • Assesses the patient/significant other’s response to teaching plan to evaluate the effectiveness of the plan.

  • Based on reassessment, plan of care will remain the same or be modified to meet the patient/significant other’s identified needs.

  • Documents outcomes of patient care.

6.    Maintains up?to?date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team, to ensure quality care.

  • Nursing assessment is complete and accurate per Would Care Center standard.

  • Each identified problem must be addressed using narrative form or Wound Care Center standard.

  • Wound Care Center record/form accurately reflects nursing interventions and medical plan of care.

  • Discharge plan and documentation accurately reflects patient and/or significant other’s ability to manage care after discharge. Referrals are made based on identified needs.

  • Interagency communication referral forms are complete and accurate.

  • Documents use of narcotics according to Wound Care Center / Hospital policy and procedure.

7.  Maintains current knowledge/certification/licensure. Pursues professional growth and development.

  • Practices within legal boundaries of nurse practice act and according to Wound Care Center / Hospital policy and procedures.

  • Attends continuing education programs in order to maintain licensure. Attends at least one seminar per year in your area of practice.

  • Attends Wound Care Center Hospital sponsored training programs as required by department.

  • Maintains Hyperbaric Training and Competencies

8. Functions as a role model for current and new staff.

  • Accepts assignment as preceptor for new staff.

  • Facilitates problem resolution among peers as observed by Wound Care Center Program Manager and peers.

  • Demonstrates a constructive approach during all interactions with staff and Wound Care Center Program Manager toward the Organization.

9. Performs all responsibilities/duties required by the Wound Care Center as defined in the scope of service to ensure that the unique nature of the client is addressed.

  • Demonstrates the ability to rapidly assess the Wound Care Center patient, differentiating between long- and short-term needs.

  • Patients are discharged from Wound Care Center with consideration given to age and mobility.

  • Discharge instructions are thorough and clear. Signatures obtained as per policy.

10. Attends all required Safety Training programs and can describe his/her responsibilities related to general safety, Wound Care Center/service safety, and specific job?related hazards.

  • Attends safety education programs.

  • Provides complete and accurate responses to safety questions.

  • Maintains work area and equipment in condition required by Wound Care Center / hospital standards.

  • Demonstrates proper body mechanics in all functions.

  • Effectively utilizes unit manuals as resources in order to clarify and/or resolve Wound Care Center / Hospital policy issues.

11. Follows the Wound Care Center / Hospital Exposure Control Plans/Bloodborne and Airborne Pathogens.

  • Demonstrates knowledge of techniques, procedures, and correct use of protective barrier equipment.

  • Attends annual education programs.

  • Wears and/or uses appropriate safety equipment.

12. Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

  • Interacts with all of the above in a considerate, helpful, and courteous manner as observed by Wound Care Center Program Director and peers.

  • Fosters mature professional relationships with fellow employees in a courteous, friendly manner as measured by management observation and peer input.

  • Maintains professional composure and confidence during stressful situations.

  • Maintains open communication using appropriate chain of command regarding issues.

  • Conducts all work activities with respect for rights and wishes of patients, visitors, families and fellow employees.

  • Maintains confidentiality of all Wound Care Center / Hospital and patient information at all times.

  • Presents neat appearance in proper attire and identification as required by Wound Care Center / Hospital policy.

  • Displays a positive attitude which contributes to the overall customer service program in place.

13. Promotes effective working relations and works effectively as part of Wound Care Center team to facilitate the centers ability to meet its goals and objectives.

  • Participates in staff meetings once/quarter.

  • Supports the Wound Care Center /  Hospital’s mission, policies, and programs through attendance and participation at committee meetings. 

  • Demonstrates ability to use the computer system.

  • Completes work assignments on time/readily accepts assignments as observed by Wound Care Center Program Manager.

  • Reports to work on time and is at work as scheduled, as observed by Wound Care Center Program Manager.

14. Assumes all other duties and responsibilities as necessary.

  • Demonstrates willingness to identify and/or assumes activities relative to the developmental needs of the unit and the nursing department.

  • Demonstrates dependability/flexibility in meeting scheduling needs of the Wound Care Center and Hospital.

    Participates in CQI (Continuous Quality Improvement) activities as assigned.


Internal and external contact with all necessary parties. Stand/Walk 6-8 hours. Sit 1-2 hours. Crawl, climb, carry, push, pull up to 1/3 of the time. Bend, squat, reach, lift 1/3 or more of time. Weight Lifted/Force Exerted up to 50lbs. Use hands or feet for gross motor function and precise motor function (or fine manipulation). Body fluid exposure. Handles multiple priorities, independent discretion/decision making, manages stress appropriately, works alone effectively, works in close proximity to others and/or in a distracting environment, works with others effectively. Attends all mandatory education programs and can describe his/her responsibilities related to general safety and regulatory compliance. Attends all required department-specific education and training programs and can describe his/her responsibilities related to department safety and specific job?related hazards. Follows the hospital exposure control plans/bloodborne and airborne pathogens. Attire is professional, neat, clean, and appropriate for the work environment. Wears proper identification while on duty. Maintains confidentiality of all hospital and patient information at all times, as observed by peers and management. Reports to work on time and meets acceptable standard for attendance. Attends a minimum of two staff meetings, reads minutes for all staff meetings, and participates in committees as required. Current registration in the state. Wound Care Certification preferred or certification within 18 months. Maintains and demonstrates competency in BLS (Basic Life Support), infection control, safety and all unit required skill review. Must be able to use Microsoft Outlook, Word, PowerPoint, and Excel, as well as webbased database programs and the Internet. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plan and solid geometry and trigonometry.  Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.  Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Hyperbaric Training and Competencies maintained. – Each chamber operator attends, or provides verification of attendance to, and introductory hyperbaric training course as approved by the National Board of Diving and Hyperbaric Medical Technology (NBDHMT). The incumbent in this position is not required to work under any extreme conditions, but may be required to work around caustic chemicals approximately 5% of the time, and must be able to work around a moderate noise level, such as an office environment.




Bachelor’s degree from a four-year college or university; or two years related experience and/or training; or equivalent combination of education experience.



Medical Surgical, home health or wound care preferred.
